Physician's Prescription
The client's physician needs to write a prescription for the requested device. The prescription should include:
- The patient/client's name
- The specific device - Mercury, MiniMerc, LinkCLASSIC, LinkPLUS, L*E*O
- All accessories or peripherals needed to use the device effectively (switch, wheelchair mount, moisture guard, software, etc.)
In addition, a letter from the doctor stating that he or she agrees with the results of the evaluation and recommends the device is helpful. In cases where the doctor is not familiar with assistive technology and the funding, the therapist may wish to prepare a letter for the doctor to sign as a convenient alternative to the doctor composing his or her own letter.
