Letter of Speech Language Evaluation
The Letter of Speech Language Evaluation is the most detailed and involved portion of the Funding Packet. This letter needs to be written by a licensed speech pathologist.
Some of the required information for the speech language evaluation includes:
- An introduction to the client: age, residence, assistance required for daily living, school or employment information, etc.
- An explanation why the current method of communication is inadequate
- Reasons why the client needs to be able to communicate, and with whom
- How the disability or need for treatment will be reduced with the requested device
- The amount of expected improvement that use of the device will provide
- How the device treats the client's condition
- Reasons why this particular device is the most effective one for the client
- A recommendation for the requested device as being the most appropriate choice for the client based on features and price
