Product Order Form
A Product Order Form will be included with the Funding Packet you request. To completely fill out the Order Form, you will need to know:
- Bill To name, organization, address, telephone and fax
- Ship To name, organization, address, telephone and fax (if different)
- Method of Payment (credit card, check or purchase order from an organization)
- Product quantity, item number, description and price
- County and state where purchase is made
Included on the form are instructions to help you assess Shipping and Handling charges.
